How to find the form code and scored-question totals

Read the Scoring Key footer. The complete form code is printed there. Do not infer a test from a URL, a “Practice Test 2” filename, or the trailing digit of QU04003-N.
Exclude items marked “Not Scored”. Field-test items do not count toward the 1–36 conversion. English, Math, Reading and Science scored maxima on these four forms are 40, 41, 27 and 34.
In a TestNav score summary, use scored questions correct — not the displayed question count (50 / 45 / 36 / 40) and not a scale score you already converted by hand.
